Registrations with professional bodies

                             As a therapist I am affiliated with the Dutch Professional Association of Hypnotherapists (NBVH) with registration number 19025.                                                       See also: http://www.hypnotherapie.nl/

My practice is registered under the name Anto Jadresko with the national information centre of health insurers, Vektis, and is included in the General Data Management Health Care Providers, AGBZ, with the personal AGB code 90107784 and the practice AGB code 90066877.

The practice is registered at the Chamber of Commerce (KvK) under the name Anto Jadresko with the KvK number 76203786.

I like to hear from you if you are not satisfied with my approach, something I have said or have not said or done. 
This can be done during the session or afterwards. We can talk and find a solution together. 

If we cannot reach a solution together, you can contact my professional association (www.hypnotherapie.nl). There you can appeal to the klachtfunctionaris, a complaints officer, as prescribed in the Care Quality, Complaints and Disputes Act (Wkkgz). I am also affiliated with an independent dispute resolution organisation, namely the SCAG (https://www.scag.nl). 

I am recorded in the Register of Professional Practitioners Complementary Care (RBCZ), with license number 109708R. This umbrella organisation takes care of the disciplinary law (see also: http://www.tcz.nu/).
